Clinical Appilication of Autologous Adipose Tissue derived Stem Cells : The Effect of Intracoronary Administaration on Acute Myocardial Infarction in Porcein Models, Adipose Tissue-derived Stem Cell Implantation in Patients with Critical Limb Ischemia

Background : Adipose derived stem cells(ADSCs) are known to be potential and effective in damaged tissue regeneration. AMI study was designed to assess the effectiveness of intracoronary administration of ADSCs in AMI in porcein model. CLI study was designed to determine safety and efficacy of multiple intramuscular ADSCs injections in critical limb ischemia(CLI). Methods : AMI study in a porcein model was studied. autologuos two million ADSCs were intracoronary injected after AMI induced by balloon occlusion in the mid-LAD for 3 hours. CLI study included 15 male CLI patients with nonhealing ulcers and necrotic foot.Results: AMI study-Analysis of 99mTc MIBI-SPECT data obtained at the baseline and after 28 ± 3 days showed that perfusion defect and LVEFimproved in the ADSCs group (n=7) compared with control group (n=7). Glucose uptake and perfusion defect was improved in ADSCs group by PET-CT analysis. The infarct area and the vascular density of border zone were improved in the ADSCs group(Table). The ADSCs identified by SDF-1 staining were well co-localized by vWF,Troponin-T staining. CLI study-Intramuscular ADSCs injections cause no complications. Clinical improvement occurred in 66.7% of patients. At 6 months, significant improvement was noted on pain rating scales and in claudication walking distance. DSA before and 6 months after ADSCs implantation showed numerous vascular collateral networks(Fig).Conclusion : Intracoronary administered ADSCs improved myocardial remodeling and function in the porcein AMI model. And, multiple intramuscular ADSCs injections may be a safe alternative to achieve therapeutic angiogenesis in patients with CLI.
