Difference of Central Hemodynamic Parameters According to The Gender in Hypertensive Patients

Objectives:Central pressures are pathophysiologically more relevant than peripheral blood pressures for the pathogenesis of cardiovascular disease and central hemodynamic parameters have been shown to be independently associated with organ damage and incident cardiovascular disease. But, until now, studies about analysis of central hemodynamic parameters according to the gender in hypertensive patients were rarely performed in korea. Methods:We recruited serial 547 hypertensive subjects who came to our cardiovascular outpatient facility from June, 1st, to July, 1st, 2010. Subjects were composed of 300 males and 247 females. The mean age was different between males and females(male 64.6±11.5, female 66.7±11.2, p=0.037). We obtained radial arterial wave from radial tonometry(HEM9000-AI) and analyzed systolic blood pressure 2(SBP2),peripheral augmentation index(pAI), peripheral augmentation index75(pAI75), systolic blood pressure1-systolic blood pressure2(SBP1-SBP2) and central pulse wave(cPP) according to the dermographics and clinical chacteristics. Results:1.Smoking, coronary heart disease and chronic kidney disease are significantly higher in the males than in females. 2.There was no difference of anti-hypertensive medication between in the males and in females. 3.SBP2, A, AI75 and central PP were significantly higher and SBP1-SBP2 was significantly lower in the females than in males. 4.In multivariate analysis including age, SBP2(p=0.028), AI(p=0.000), central PP(p=0.096) and SBP1-SBP2(p=0.000) were independently different according to the gender. Conclusions:Although female hypertensive patients had less cardiovascular risk factors such as smoking, coronary heart disease or chronic kidney disease, they showed the more increased arterial stiffness parameters of central hemodynamics.
