Long term (5-Year) Outcome after Drug-Eluting Stent Implantation in Non-bifurcation Lesions Involving Unprotected Left Main Coronary Artery; Data from A Multicenter Registry

Objective The aim of this study was to evaluate the long-term clinical outcomes of drug-eluting stent(DES) implantation for ostial and shaft lesion of unprotected left main coronary artery disease(LMCA). Background Limited data are available on the long-term safety and efficacy of DES implantation for patients with non-bifurcation lesion of LMCA disease. Methods We evaluated 339 consecutive patients who underwent stenting with DES in the ostium and/or the shaft lesion. The 5-year outcomes including death, the composite outcome of death, Q-wave myocardial infarction (MI), or stroke, and target vessel revascularization (TVR) were investigated. Results Nonbifurcation lesions were treated with sirolimus eluting stents (n=254, 74.9%), paclitaxel eluting stents (n=81, 23.9%) and zotarolimus eluting stents (n=4, 1.2%). The median follow-up was 54 months (interquartile range, 45.1 to 63.5 months) and complete follow-up for major clinical events was obtained in 98.2%. During overall follow-up, 38 patients (11.2%) died. A composite of death, Q-wave MI, or stroke occurred in 40 patients (11.8 %). TVR was performed in 37 patients (10.9%). In multivariate analysis, age (hazard ratio [HR], 1.054; 95% confidence interval [CI], 1.022-1.088; p<0.001), congestive heart failure (HR, 5.906; 95% CI, 2.235-15.603; p<0.001), and CRF (HR 4.713; 95% CI, 1.615-13.750; p=0.005) were identified as the independent predictors for serious composite outcome.(Table1) Kaplan Meier curve showed more frequent TVR in patients who need bifurcation stenting than nonbifurcation. Conclusions During 5-year of follow-up, DES implantation for ostium and/or shaft LMCA lesions appears to be safe and feasible.
