Is overweight and obesity itself independent risk factor for hypertension?

Background: It is well known that obesity is the independent risk factor in cardiovascular disease, especially hypertensive patients, also other risk factors such that non-dipper type hypertension, target organ damage (microalbuminuria, LVH), high CRP high augmentation index and pulse wave velocity (PWV) are well known in hypertension. We hypothesize that overweight and obesity which evaluated by only BMI are not enough to evaluate risk factor for HTN, therefore other co-factors (such as lipid profiles) will be affect to risk stratification. Method and Result: From November 2007 to February 2009, 100 patients were newly diagnosed as essential hypertension in our OPD, all patients were performed 24 hours ambulatory BP monitoring, ECG, Echocardiogram, Laboratory test, ABI and PWV test. Normal BMI patients (n=20), insufficient test and peripheral artery disease were exclude (n=9). 71 Patients were overweight(23≤BMI<25) or obesity(BMI≥25) and divided two groups, Group I was less than 130mg/dL of serum LDL cholesterol level (n=48), group II was more than 130mg/dL of serum LDL cholesterol level(n=23). There were no significant differences of age, gender, BMI, mean BP, mean HR, day and night time BP, degree of differ, CRP level, urine albumin/creatinine ratio, LVEF, LVMI(evaluated by echocardiography), ABI, augmentation index, ABI at right and left, baPWV, faPWV at right and left. Only heart to femoral PWV(HF PWV) was significantly faster in Group II (Group I vs Group 2; 921.13±163.81cm/sec Vs 1044.68±182.89cm/sec, p=0.012), also LDL cholesterol level was positive correlation with HV PWV(R=0.363, p=0.005), but heart to carotid PWV was not significant difference (Group I vs Group 2; 927.45±265.98cm/sec Vs 1046.15±261.97cm/sec, p=0.107) and correlation of each others was not noted. Conclusion: Aortic PWV and augmentation index are independent predictors of adverse cardiovascular events including mortality. Overweight and/or obesity are traditional risk factor for hypertension. But other papers suggest that obesity paradox is existed at now in cardiovascular disease. Our data suggest that overweight and/or obesity based on only BMI do not reflect aortic PWV, overweight and/or obesity with high LDL-cholesterol level is more potent risk factor for hypertension and reflection of aortic PWV. Therefore more detail analysis will be required for the role of overweight or obesity in the risk stratification of hypertension.
