Impact of Age on Mid-term Angiographic and Clinical Outcome of Percutaneous Coronary Intervention with Drug-eluting Stents in Alcoholic Patients

Background: Age has known to be a strong risk factor of coronary artery disease (CAD). However, whether alcohol consumption is significantly associated with worse outcomes after the percutaneous coronary intervention (PCI) in drug-eluting stent (DES) era is still in controversy. We investigated the impact of age on the mid-term angiographic and clinical outcomes after PCI with DES only in pts with chronic alcohol consumption.Methods: A total 551 consecutive alcoholic patients (pts) underwent PCI with DESs between 2004 and 2008 were enrolled for the analysis. Pts were divided into two groups according to the age (Old age: ≥ 60 years, n=303 pts, Young age: <60 years, n=248 pts). Six-month angiographic outcomes and 12-month clinical outcomes were compared between the two groups.Results: Baseline characteristics were not different between the two groups except diabetes (37.4% vs. 25.2%, p=0.02), cerebral vascular disease (9.3% vs. 1.3%, p<0.01), and peripheral vascular disease (13.7% vs. 4.6%, p=0.04) were more common in old age group. At 6 months, the incidence of angiographic binary restenosis was higher in the elderly alcoholic pts group. However, major clinical outcomes comprising cardiac death, Q-wave myocardial infarction (MI), target lesion and vessel revascularization (TLR & TVR) and major adverse cardiac events (MACE) were not different between the two groups up to 12 months Conclusion: When we consider that the elderly pts are at risk of worse angiographic and clinical outcomes after the PCI with DES, however, the age itself in pts with chronic alcohol consumption did not show a negative impact on the major clinical outcomes up to 12 months, suggesting old age itself cannot be a major limitation factor in alcoholic pts who need coronary revascularization.
