Sirolimus-eluting Stent versus Non-Sirolimus Drug-eluting Stents in ‘Real World’ Unprotected Left Main Intervention |

Background: Sirolimus-eluting stent (SES; Cypher) has recently been known to be associated with profound delayed healing and intimal suppression with better angiographic outcomes compared with those of non-sirolimus drug-eluting stents (DES). We compared the safety and efficacy of SES with those of other DESs in unprotected left main (LM) angioplasty up to 12 months.
Methods: All patients (pts) with unprotected LM lesions were electively treated with sirolimus-(SES; Cypher), paclitaxel-(PES; Taxus), zotarolimus-(ZES; Endeavor) or everolimus-(EES; Xience) eluting stents. LM bifurcation lesions were treated by kissing stenting, crushing technique or stent crossover (Stent/Balloon). Clinical outcomes at 12 months and angiographic outcomes at 6 months were assessed.Results: Out of total 120 pts (73 males, mean age:64.1 ± 9.9 years), 117 pts underwent standard percutaneous coronary intervention (PCI) with DES on unprotected LM lesion, 68 pts (68/117, 58.1%) were exclusively treated with SES, 49 pts (49/117, 41.9%) with non-SES (31 PES+16 ZES+2 EES). There was a trend toward less incidence of binary restenosis at 6 months and lower incidence of repeat PCI in LM alone lesion in the SES group up to 12 months (Table). Conclusion: Although there was a trend toward less binary restenosis and repeat PCI in LM alone lesion in the SES group as compared with non-SES group, these differences were not translated into different major clinical outcomes up to 12 months.
