The role of calcium score in asymptomatic individuals with significant lesions detected by coronary CT angiography

Object: The purpose of this study was to define the cutoff value of the coronary artery calcium score (CACS) of the asymptomatic patients who need the revascularization such as percutaneous intervention (PCI) or coronary arterial bypass graft (CABG). Method: We reviewed 840 medical records of asymptomatic subjects who underwent computed tomographic angiography (64-slice multidetector row computed tomography ; MDCT) as part of a general health evaluation. Results: Atherosclerotic plaques were identified in 332 (39.3%) subjects. Sixty (7.1%) individuals had significant diameter stenosis (≥50%). Among them, 28 patients received conventional coronary angiography. Seventeen patients received PCI, 3 received CABG, and 8 were referred to medical treatment. The cutoff value of CACS that could discriminate between groupings of those who received revascularization or conservative treatment was 111.0 (AUC=0.87, 95% confidence interval; 0.76-0.97, sensitivity 80.0%, specificity 92.1%). The prevalence rate of significant diameter stenosis was significantly higher in the group who had more than a 10% risk of coronary heart disease (CHD) at 10 years (p>0.05). Conclusion: If the CACS of asymptomatic patients who had significant stenotic lesions on MDCT were more than 110, revascularization as a therapeutic strategy might be needed. MDCT would be more effective when applied to asymptomatic patients with more than a 10% risk of CHD at 10 years.
