Transradial coronary intervention: Comparison of the Left and Right radial approach

Background: In transradial aprroach for coronary artery graphy, the course of the guiding catheter can be affected by the brachiocephalic artery and subclavial artery, and vascular tortuosity is increased with increasing age. We express vascular tortuosity with aging numerically and compare left transradial approach with right transradial approach by age, so try to find most useful approach site in special groups. Method: From March 2008, we performed the transradial coronary intervention for patients visited in St. Gallolo Hospital by reason of coronary symptom and with normal Allen's test. (left approach:58 cases,right approach:72 cases) We evaluated the degree of axillary tortousity(good,mild,moderate,severe), angle of aorta to peripheral artery(mild,moderate,severe), procedure success rate, size of radial artery(small,large), procedure convenience(easy,hard), torque handling(easy,moderate,hard) and vascular complication of both groups. Results: Age was increased as the degree of axillary artery tortousity and the angle of aota to peripheral artery was increased. There was no difference in the procedural success rate(Lt. group 94.83%, Rt. group 95.65%,P=0.8273)and procedure convenience(easy 54 hard 1 in Lt. group, easy 63 hard 3 in Rt group,p=0.4034).There was no severe vascular complication in the both group. In patients over 65 years , the degree of axillary tortousity in the left group was smaller than that of the right group(Lt. group good 12 mild 8, Rt. group good 5 mild 12 moderate 3,p=0.035),and the angle of aorta to peripheral artery in left group was more mild than that in right group(Lt group mild 18 moderate 2, Rt group mild 7 moderate 13, p=0.0003). But there was no difference in procedural success rate(Lt. group 95.24%, Rt. group100%,p=0.3354)and procedure convenience(easy 20 hard 0 in Lt. group, easy 17 hard 2,p=0.1363) Conclusion: In patients over 65 years, the degree of axillary tortousity and the angle of aorta to peripheral artery was decreased in left radial approach, but the fact did not effect caronary artery graphy success rate and convenience statistically.
