Clinical experience and initial results of CTO intervention by retrograde approach

Background: Inability of wire crossing still remains the main cause of procedure failure of CTO intervention and retrogade approach was recently introduced to overcome this problem. The purpose of this study is to evaluate the feasibility and initial experience of CTO intervention by retrograde approach. Method: We investigated the CTO-PCI cases from August 2006 to July 2007 and analyzed retrograde approach cases among them. Results: For one year, 48 cases of CTO underwent PCI with success rate of 89.5%(43/48). Among them, retrograde approach was attempted in 33% of cases(16/48) with 75% of success rate(12/16). In failed retrograde cases, guide wire couldn’t cross collateral channels and 2 cases were switched to antegrade approach with successful result. Detailed results are shown in the table. There were no deaths or major cardiac complications. Conclusion : Retrograde approach can be considered as effective and safe method for CTO intervention and it needs to be systemized in the near future.
