Insulin resistance is associated with
arterial stiffness in nondiabetic hypertensives
independent of metabolic status

Background: Arterial stiffness is an independent risk factor for cardiovascular disease, and insulin resistance has been known to be associated with increased arterial stiffness in patients with DM, metabolic syndrome. We sought to determine that insulin resistance is related to arterial stiffness in nondiabetic hypertensive patients and this relationship is independent of metabolic status and gender.
Methods: We enrolled 284 non-diabetic hypertensive patients being treated. At the initial enrollment, the patients underwent baseline laboratory assessment including HOMA IR index and PWV. HOMA IR index is used as a marker of insulin resistance and baPWV is used as a marker of arterial stiffness.
Results: Of the 284 study subjects, 121 were classified as metabolic syndrome and 163 as non metabolic syndrome. The average baPWV was significantly higher in patients with metabolic syndrome (1506±235 vs. 1435±211 cm/sec, p=0.009). HOMA index was still independently associated with increase in arterial stiffness(r=0.555, p-value<0.001) after controlling for age, systolic BP, BMI, TG level, HDL level.(Table) The independent association of HOMA with arterial stiffness was demonstrated in subgroup analysis regardless of the metabolic status and gender
Conclusion : Increased insulin resistance was associated with arterial stiffness independent from age, blood pressure and risk factors associated with metabolic syndrome. This independent association of insulin resistance was demonstrated regardless of gender and metabolic status.
