Stenting of Left Main Coronary Artery Stenosis with Drug-Eluting Stent (DES):Korean Muticenter Angioplasty Team (COMATE) Registry

Background: Several studies reported the safety and effectiveness of stenting of left main coronary artery (LMCA) stenosis with bare metal stent. However, in-stent restenosis (ISR) still remained as a major problem in this lesion. We investigated the safety and effectiveness of DES in LMCA. Method: Thirty-eight consecutive patients (59±10 years old, men: 27) with symptomatic LMCA stenosis had stenting with 39 DES (20 Cypher and 19 Taxus). Immediate results, follow-up clinical and angiographic outcomes were evaluated. Results: Twenty–seven patients (70%) were diagnosed as acute coronary syndrome, while 12 patients (30%) were with stable angina. Twenty-two procedures (56%) were performed for the LMCA only, while 17 procedures (44%) were performed for the LMCA and proximal LAD or LCX. In quantitative coronary angiogrpahy, lesion length was 15.6±10.0mm, reference diameter was 3.4±0.7mm, minimal luminal diameter was 0.7±0.6mm (before) and 3.2±0.5mm (after). The procedures were technically successful in all cases. Clinical and 6 month-angiographic follow-up were done in 37 cases (95%) and 9 cases (24%) respectively. During the follow-up (5.5±3.2 months), 3 patients (8%) died (2 cardiac, 1 non-cardiac). Follow-up angiography revealed minimal luminal diameter of 2.5±0.6mm and late loss of 0.6±0.4mm. Conclusion: Stenting of LMCA with DES showed favorable clinical and angiographic outcome. However, data of long-term follow-up in large number of patients are still limited.
