Background and methods: Despite of recent advances in the management of acute myocardial infarction, major adverse cardiac events (MACEs) in any periods are continuous concern especially for diabetic patient. We are tried to analyze data of patients with diabetes mellitus (n=2,064) in-hospital, 1 month, 6 month and one year MACE from registry database of multicenter trial (KORMI : Korea Working Group on Myocardial Infarction). Result: Baseline characteristics showed male predominance (Male 1,312 patients, 63.6%). Atypical symptomatic presentations on admission are more frequent in female gender (27.6% vs. 22.6%, p=0.012) and history of hypertension is more frequent in male gender (38.9% vs 23.5%, p<0.001). Initial blood glucose level was higher (238.7± 111.2 vs. 225.8±99.1, p=0.011) and reference vessel diameter was smaller in female gender (2.97± 0.37 vs. 3.11± 0.50, p<0.001). Ejection fraction was not different between groups (50.7± 12.4 in male vs 50.2± 13.1 in female, p=0.42). As for in-hospital mortality, history of hypertension (HR 2.48, 95% CI: 1.08 to 5.67, p=0.032), ejection fraction (HR 0.93, 95% CI: 0.90 to 0.96, p<0.001) are independent predictors in these patients. However, one to twelve months of major cardiac events were not noticeable according to the clinical characteristics or angiographic findings. Conclusion: Of these diabetic patients with acute myocardial infarction, some clinical risk factors and angiographic parameters have some gender differences. Interestingly, history of hypertension and low ejection fraction would be potential predictors for in-hospital mortality in patient with acute myocardial infarction of diabetic Korean population.
