Relation of left ventricular and left atrial synchronicity in hypertensive patients with preserved systolic function

Background: Previous studies have assued that LV systolic dyssynchrony and diastolic dyssynchrony had different developing mechanism in diastolic heart failure. However, it is unclear whether such findings exist in hypertensive patients who have normal systolic function. And the relation of atrial and ventricular synchronicity was rarely not examined. The purpose of this study was to determine relation of ventricular and atrial synchronicity in hypertension patients with preserved systolic function. Method: Consecutive138 hypertensive patients with preserved systolic function (LVEF>50%) and 27 normal controls were included for this study. From the simple pulsed wave Doppler on the septal and lateral mitral annulus, we obtained dyssynchrony index. Left ventricular systolic and diastolic dyssynchrony index was determined as the difference of time interval from QRS onset to peak systolic velocity and peak early diastolic velocity between septal and lateral mitral annulus. Atrial dyssynchrony index was determined as the difference of time interval from P wave onset to peak atrial systolic velocity between septal and lateral mitral annulus. Result: There was no significant correlation between LV systolic and diastolic dyssynchrony index, however, atrial dyssynchrony was weakly associated with both systolic dyssynchrony index (R=0.297, p<0.001) and diastolic dyssynchrony index (R=0.177, p=0.037). LV systolic dyssynchrony index was related with E/E’ ratio (R=0.297, P=0.021). LV diastolic dyssynchrony index was related to deceleration time (DT)(P=0.008) and LV mass index (LVMI)(P<0.001). Left atrial dyssynchrony was related to LA volume index (P=0.017), DT(P=0.006) and E/E’(P=0.058). Conclusion: Systolic dyssynchrony index is not associated with diastolic dyssynchrony index. However, atrial dyssynchrony are related both diastolic and systolic dyssynchrony index. Atrial synchronicuty may be influenced systolic and diastolic synchronicity in different ways.
