Effects of combined Telmisartan with Rosuvastatin on Endothelial Function and Arterial Stiffness in Hypertensive Patients

Background and Objectives: Several studies have shown that the angiotensin receptor blockers (ARBs) improve endothelial function and arterial stiffness. It has been reported that statins, 3-hydroxymethylglutaryl-CoA (HMG-CoA) reductase inhibitors, have salutary effects on lipid profile and modulation of atheromatous plaque biology in many risk groups. The purpose of the present study is to evaluate the additive effects of rosuvastatin to telmisartan on endothelial function, insulin sensitivity, and arterial stiffness in the patients with hypertension. Subjects and Methods: Fifty patients with mild to moderate hypertension were administered telmisartan 80 mg once daily for 8 weeks using an open-labeled and prospective protocol. And then, twenty-six of them were to continue telmisartan therapy alone and twenty-four of them were administered 20 mg dose of rosuvastatin once daily combined therapy with telmisartan for 8 weeks. The patients were examined at baseline, 8 weeks and 16 weeks follow-up visits to assess changes of flow mediated-vasodilation (FMD), QUICKI (quantitative insulin–sensitivity check index), HOMA (homeostasis model assessment), adiponection and pulse wave velocity (PWV). Results: Telmisartan therapy increased FMD from 7.653.3% to 9.573.1% (p<0.01) at 8 weeks follow-up. Most arterial stiffness parameters were improved after telmisartan therapy. But, there was no significant change in QUICKI, HOMA level (p=0.188, p=0.091) and plasma adiponectin (p=0.227). At 16 weeks follow-up, FMD was increased in whom received telmisartan alone (7.473.1 to 9.932.9 %, p<0.01) and also increased in whom received the rosuvastatin added to telmisartan (7.843.5 to 10.473.5 %, p<0.01). But there were no significant visible difference between the telmisartan single therapy and telmisartan and rosuvastatin combined therapy. Conclusion: These results suggest that telmisartan is effective in improving endothelial function and arterial stiffness in patients with essential hypertension. But adding rosuvastatin to telmisartan had little additive effect on that.
