Clinical Usefulness of Myocardial Contrast Echocardiography for detecting Stress induced Cardiomyopathy

Background and Objectives : Stress induced cardiomyopathy(SCMP) is characterized by an acute onset of reversible left ventricular (LV) dysfunction in the absence of significant coronary artery disease. Most patients present with signs and symptoms often confused with those found during an acute anterior wall myocardial infarction. Myocardial contrast echocardiography(MCE) provides the potential of real time, noninvasive assessment of myocardial perfusion at the bedside. The purpose of this study is to investigate the clinical usefulness of MCE on detection of SCMP. Methods : We investigated 46 patients (61 ± 13 years, 22 famales) who visited emergency department for chest pain, elevated cardiac enzyme and had wall motion abnormality at apex by 2D echocardiography. All of these patients were assessed by real time MCE using PESDA and following parameters were measured: A represents myocardial blood volume, β represents the mean microbubble velocity, and A and β represents myocardial blood flow(MBF). After MCE, we performed diagnostic CAG in all patients. Results : Of 46 patients, 31 patients had significant perfusion defect at apex(group A) and 15 patients had no perfusion defect or slightly delayed perfusion defect at apex(group B) by MCE. There were significant differences in quantitative MCE parameters between 2 groups (table). In group A, all patients showed significant luminal stenosis or thrombus in LAD. However, in group B, 13 patients showed no luminal stenosis and only 2 patients showed moderate stenosis in LAD. The results of quantitative MCE parameters were shown in the table. Conclusions : The data from this study shows that myocardial perfusion measured by MCE is relatively preserved in patients with SCMP than myocardial infarction. Therefore, real time MCE may be a useful noninvasive diagnostic tool for detecting SCMP.
