Vascular response at the edges of drug-eluting stents

Background: Drug-eluting stents (DES) have shown dramatic reduction in in-stent restenosis and target lesion revascularization. However, vascular response at the edges of DES could limit their effectiveness. The aim of this study is to compare the vascular response using intravascular ultrasound (IVUS). Methods: Two hundred six edges were analyzed in 103 patients treated with DES and divided in sirolimus-eluting stents (71 patients) and paclitaxel-eluting stents (32 patients). IVUS was performed post-intervention and at 9 month follow-up and included 5mm distal and proximal to the stented segment. Results: Baseline characteristics were similar between two groups. The rate of edge restenosis at follow-up showed no statistical significance (1.4% in SES vs. 9.4% in PES, p=0.088). There were no significant differences in vessel volume of proximal and distal edges in SES compared with PES. However, there was significant increase in vessel volume in distal edges in SES group (10.36±4.39 mm3/mm vs. 11.29±4.33 mm3/mm, p<0.001). Conclusions: Compared with PES, SES showed significant positive vascular remodeling in distal edge. Although different vascular response was observed, restenosis rate was equivalent between two groups at 9-mont follow-up.
