Clinical Effects of Statin in Elderly Patients with Acute Myocardial Infarction and Left Ventricular Failure after Drug-Eluting Stent Implantation

Background and Objectives: Recent studies show use at the time of heart failure (HF) hospital discharge significantly increased prognosis and survival rates. We examined the early effects on major adverse cardiac events (MACE) of statin use at discharge in patients hospitalized for elderly AMI patients with LV failure after drug eluting stent (DES) implantation compared with outcomes in patients who do not receive before discharge. Subjects and Methods: From 2005 to 2008, among 1234 patients with AMI admitted in our hospital, we selected elderly patients (≥70years old) with AMI with LV failure (EF≤50%) treated with DES and then we divided into two groups: one is group who received stain before stenting, the other is group who did not receive any statin. We analyzed 6 month follow-up MACE after discharge. Results: There was 202 patients with elderly AMI patients with LV failure and 178 patients was treated with DES among them 137 patients received statin and 41 patients did not receive statin. There were no significant differences in age, sex ratio, blood pressure, heart rate on admission, ejection fraction and lipid profiles. There were no significant differences in history of ischemic heart disease and prevalence of diabetes, hypertension and dyslipidemia. According to multiple logistic regression analysis, statin use significantly reduced 6-month MACE (p=0.026, HR=0.105, 95% CI: 0.014-0.764). Conclusion: Predischarge statin improved clinical outcomes in elderly AMI patients with LV failure after DES implantation.
