Background: The black-hole (BH) is intraluminal tissue with a homogenous echolucent appearance that produces no attenuation of the ultrasound signal. Almost BH phenomenon was reported after sirolimus-eluting stent (SES) implantation. We evaluated the incidence and characteristics of BH after drug-eluting stent (DES) implantation. Methods: The patients were enrolled from intravascular ultrasound (IVUS) databases of two hospital centers. We evaluated clinical characteristics and volumetric analysis was performed for vessel (VVI), lumen (LVI), plaque (PVI), neointima (NIVI) and BH (BHVI). Results: Among 242 patients, a total of 9 patients (10 sites, 3.72%) who showed BH phenomenon at follow-up IVUS were evaluated. The mean length of BH was 4.08 짹 3.37 mm and BHVI was 0.25 짹 0.27 mm3/mm. Burden of BH (ratio of BHVI and NIVI) was 42.19 짹 32.96 %. However, BH was not associated with adverse cardiac events (follow-up duration: 276.60 짹 59.34 days). Conclusions: The BH was observed in everolimus-eluting stent and paclitaxel-eluting stents as well as SESs. Because the amount of BH was small and the length was focal, the clinical course of BH was benign. Further investigations will be needed to define clinical consequences associated with the BH phenomenon.
