Improvement of coronary flow reserve (CFR) with feet immersion to hot water in patients with coronary artery disease

Backgrounds: Recent studies have shown that thermal therapy by means of warm-water baths and sauna has beneficial effects in chronic heart failure. But warm-feet bath has generally been considered inappropriate for patients with coronary artery disease. However, a comprehensive investigation of the hemodynamic effects of thermal vasodilation on coronary artery has not been previously undertaken. In this study we tried to show the effect of warm feet bath on coronary artery in patients with coronary artery disease. Methods: We studied 22 patients (67 % men, mean age 59.2 ± 1.5 years) with coronary artery disease. They were performed coronary flow Doppler examination of left anterior descending coronary artery and coronary flow reserve (CFR) was measured with adenosine before and after warm feet bath. The temperature was evenly maintained at 40°C for 30 min. Results: Systolic, diastolic blood pressure and heart rate did not change with warm feet bath. Mean velocity of coronary flow significantly increases (diastolic mean flow velocity : 22.24 ± 3.19 cm/min initial, 24.95 ± 5.65 cm/min follow up, p = 0.031) and CFR significantly improved (1.45 ± 0.63 vs 2.21 ± 0.75, p = 0.008). The warm feet bath was well accepted and no relevant adverse effects were observed. Conclusion: Warm feet bath significantly improves coronary flow without any side effect in patients with coronary artery disease and it can be applied with little risk of coronary artery event if appropriately performed.
