Comparison of Clinical Outcomes after Cobalt-Chromium Stent versus Everolimus-Eluting Stent Implantation in Elderly Patients

Background: The clinical benefit of percutaneous coronary intervention (PCI) with drug-eluting stent compared with bare metal stent in elderly patients is still unclear. The purpose of this study was to compare the clinical outcomes at 12 months after everolimus-eluting stent (EES) versus cobalt-chromium stent (CCS) implantation in elderly patients over 75 years old with ischemic heart disease. Methods: We included 84 consecutive patients (39 males, age 79.6±3.9 years) aged 75 or over who had undergone PCI with EES (n=48) or CCS (n=36) at 3 medical centers. Primary endpoint was cumulative incidence of major adverse cardiac events (MACE) including cardiac death, myocardial infarction (MI), target vessel revascularization (TVR) and stent thrombosis (ST) at 12 months. Results: During 12-months follow-up period, EES group showed tendency of lower incidence of cardiac death (0.0% vs. 8.3%, p=0.075) and MI (0.0% vs. 8.3%, p=0.075). The incidence of MACE showed no difference between two groups (2.1% vs 11.1%) Conclusion: PCI with EES in elderly patients showed similar clinical outcomes as compared to PCI with CCS.
