Impact of Coronary Stent Thrombosis on Clinical and Angiographic Outcomes in Patients undergoing Percutaneous Coronary Intervention with Drug Eluting Stents in Asian Population

Background Stent thrombosis (ST) is very rare but can be a devastating complication in the drug-eluting stent (DES) era and DES ST is known to be a predictor of worse angiographic and clinical outcomes. The aim of study is to evaluate the impact of DES ST on angiographic and clinical outcomes in a series of Asian population up to 1 year.Methods A total of 2058 consecutive patients (pts) undergoing percutaneous coronary intervention (PCI) with DESs were enrolled and divided into two groups according to presence of angiographic ST (ST: n=30 pts , non-ST: n=2028 pts). ST was defined according to ARC definition as acute (< 24 hours), subacute (24 hours to 30 days), late (>30 days to 1 year), or very late (>1 year) ST. Results Angiographic outcomes at 6-month showed that restenosis % was higher in overall ST and follow up minimal luminal diameter (MLD) was smaller in overall ST. However, major clinical outcomes were not different between the two groups up to 12-month (Table). Further, late ST was an independent predictor of total major adverse cardiac event (MACE, hazard ratio [HR]: 11.2, 95% confidence interval [CI] 1.56 to 80.27, p=0.01). Acute ST ([HR]: 21.7, 95% [CI] 1.34 to 350, p=0.03) and late ST ([HR]: 22.0, 95% [CI] 3.5 to 158, p<0.01) were independent predictors of target lesion revascularization (TLR). However, overall ST was not independent predictors of TLR, TVR, and MACE.Conclusions Despite the DES ST group was associated with worse mid-term angiographic outcomes compared with those of Non ST group, major clinical outcomes up to 12 months were similar between the two groups. However, late ST was an independent predictor of TLR TVR and MACE and acute ST was for TLR. Prevention of acute and late ST may play an important role for better prognosis.
