Background: It has been known that serum uric acid (UA) is an independent predictor for cardiovascular (CV) outcomes in patients with ST-segment elevation myocardial infarction (STEMI). However, the prognostic importance of UA on CV outcome in patients with acute myocardial infarction (AMI) including non-STEMI remains unclear. We investigated the short-term prognostic value of UA in patients with AMI. Methods: In this retrospective study, we included 901 consecutive patients (599 men; mean age = 66짹11 years-old) with AMI admitted to emergency room between November 2005 and December 2007. Mean follow-up duration was 11.6 짹 4.7 months. The primary end-point was 6-month major adverse cardiovascular event (MACE) including death, myocardial infarction (MI) and revascularization. Results: During 6-month follow-up, there were 145 (17.0%) MACEs including 84 (9.8%) all cause deaths, 29 (3.4%) non-fatal MI and 32 (3.8%) revascularization. The baseline levels of UA at admission were significantly higher in patients with 6-month MACEs than in those without MACEs. In Cox proportional-hazards model, UA (hazard ratio (HR) 1.116, 95% confidence interval (CI) 1.014 to 1.228, p=0.025) in addition to log-transformed serum N-terminal pro B-type natriuretic peptide (log proBNP) (HR 2.005, 95% CI 1.321 to 3.041, p=0.001), serum sodium (HR 0.955, 95% CI 0.914 to 0.997, p=0.037) and heart rate (1.012, 95% CI 1.002 to 1.022, p=0.018) was an independent prognostic factor for 6-month MACE after adjusting age, sex, diabetes, Killip ���2, hemoglobin, potassium, triglyceride, creatinine and log transformed high-sensitivity C-reactive protein. Moreover, UA showed a better incremental prognostic value over conventional risk factors including log proBNP (global chi-square increase from 126 to 135, p=0.002). Conclusion: UA is an independent predictor for 6-month MACE in patients with AMI.
