Background: The long-term serial angiographic outcome between sirolimus- (SES) versus paclitaxel-eluting stent (PES) is unknown. Methods: From the prospective observational study assessing the 2-year serial angiographic outcomes in lesions treated drug-eluting stent (DES), 614 lesions were treated with SES and 160 with PES. The inclusion criteria were patients who received SES or PES, and had 6-month (6Mo) angiography and 2-year (2Yr) angiography. Exclusion criteria were patients who had major adverse cardiac events after 6-month angiography, serious comorbid disease, graft lesions, lesions treated with both SES and PES, and angiographic restenosis at 6-month angiography. The primary outcome of interest was the late loss (LL) classified into early (post-procedure – 6Mo), delayed (2Yr – 6Mo), and overall (post-procedure – 2Yr) periods. For adjustment of the different follow-up time, LL was divided by follow-up time (months) and multiplied by 12 to obtain LL rate per year (mm/yr). Results: Baseline demographic and clinical characteristics did not differ between the SES and PES groups. The values in the table were presented as median (interquartile range). After adjustment, SES was associated with greater LL rate in early (OR, 0.316; 95% CI, 0.213-0.469; p<0.001) and overall (OR, 0.532; 95% CI, 0.360-0.786, p=0.002) periods than PES. However, delayed LL rate was independent to the type of DES. Conclusion: A progressive luminal narrowing over 2 years was observed in both SES and PES implantation. In particular, PES experienced more reduction of 2-year luminal loss than SES, driven by more loss in the early 6 months. However, the DES type was not associated with the luminal loss between 6 months and 2 years.
