Clinical and Echocardiographic Features of Right-sided Infective Endocarditis in the Korean Population with Low Prevalence of Intravenous Drug Use

Background and Objectives: Intravenous drug use (IDU) is known as the most significant risk factor for right-sided infective endocarditis (RIE) in the western countries. The clinical situation would be different in the country with very low prevalence of IDU such as Korea. The aim of our study is to investigate clinical and echocardiographic characteristics of RIE in Korea. Subjects and Methods: Two hundred and forty two patients with Infective endocarditis diagnosed by the modified Duke criteria were analyzed from 1995 to 2009. RIE and left-sided infective endocarditis (LIE) were defined according to the valve site involving vegetation. Patients without vegetation in echocardiography or with vegetation involving both-side cardiac structure were excluded from the analysis. Clinical and echocardiographic characteristics of RIE were compared to LIE. Results: In total 242 patients, 204 patients (84.3%) had LIE and 38 patients (15.7%) had RIE. In RIE group, patients were younger (Mean age; 40±18 years vs. 50±18 years, p<0.01), had higher incidence of uncorrected congenital heart disease (39.5% vs. 13.2%, p<0.01), and central catheter (RIE; 18.4% vs. LIE; 3.9%, P<0.01). Large size (≥10mm) of vegetation was more common. Significant valvular dysfunction was less found (10.5% vs. 48.5%, p<0.01) in RIE group. Species of causative microorganism, in-hospital days and in-hospital mortality were not different between two groups. Conclusions: Uncorrected congenital heart disease and indwelling central catheter were the most important risk factors for RIE in the population with low prevalence of IDU. Its short-term clinical outcome might be same as that of LIE.
