Clinical outcome for Grown-Up congenital heart patients : 1 hospital experience in Korea

Introduction Although almost all congenital heart disease patients are treated within pediatric age, many grown-up congenital heart (GUCH) patients require surgical and interventinal correction in adult age. The objective of this study is to present the outcomes of correction of GUCH patients. Patients and Methods. We have collected data from 1032 patients who were more than 18 years and underwent cardiac operations and interventions between January, 1 1997 and December, 31 2007 in Sejong general hospital. Results The diagnosis was septal defect(62%, n=641), aortic valve disease(9.1%, n=94), patent ductus arteriosus (PDA) (10.3%, n=106), Tetralogy of Fallot (TOF) (6.3%, n=65), RVOT disease (3.3%, n=34), and etc. The common presenting symptoms was asymptomatic, exercise intolerance, dyspnea, and etc. The preoperative risk factors were present in NYHA functional class more than II, pulmonary hypertension, arrhythmia, and old age. Patients were divided into three groups: (1) palliative procedures (0.8%, n=8), operation and intervention performed to improve patients’ clinical status without restoring normal anatomy or physiology; (2) repair (90.2%, n=931), operation and intervention performed to achieve an anatomic or physiologic repair with separation of the pulmonary from the systemic circulation. It was ASD or VSD closure, total correction of TOF, aortic valve or root replacement, and Fontan operation, and etc. (3)reoperation (9.2%, n=95), all the reoperations performed after repair . It was pulmonary valve replacement, tricuspid valve replacement, conduit change, and etc. The postoperative complication rate was 9.2% (n=95). The common complication was arrhythmia (2.4%, n=25). The mortality case is rare (n=10). Conclusion Over the follow-up period, relatively good functional class and low mortality were reported. The surgical and interventional correction of Grown-Up congenital heart patients in specialized cardiac units is relatively safe and low risk.
