Mid-term Clinical Outcomes of ST Elevation Myocardial Infarction versus Non ST Elevation Myocardial Infarction Patients Undergoing Percutaneous Coronary Intervention with Drug-Eluting Stent

Backgrounds: There are very limited data on mid to long-term clinical outcomes between ST elevation myocardial infarction (STEMI) and NSTEMI pts undergoing percutaneous coronary intervention (PCI) exclusively with drug-eluting stent (DES) in a series of large number of Asian population. Method: A total of 5541 (male 72.5%, mean age 62.5±12.4 years) patients (pts) admitted with STEMI or NSTEMI and underwent successful PCI with DES implantation were studied for major adverse cardiac event (MACE) at 8 months. Pts were grouped as presenting with STEMI (n=3627 pts) or NSTEMI (n=1914 pts). Patients who did not undergo PCI, had unsuccessful PCI, implanted no DES nor no stent implantation were excluded from the study. Results: Baseline clinical characteristics were similar between the two groups. At 8 months, the incidence of MACE in STEMI group was higher compared with NSTEMI group [332/3627 (9.2%) vs. 137/1914 (7.2%) pts, p=0.011]. After the multivariate analysis including age and other conventional risk factors, the STEMI group still showed significantly higher incidence of MACE as compared with NSTEMI group [P= 0.009 , OR 1.328 with CI 1.074-1.643]. Conclusion: Despite of immediate successful PCI with DES implantation, pts presenting with STEMI may have significantly higher chances of suffering from MACE at eight months as compared to the pts presenting with NSTEMI.
