Direct Comparison between The Brachial Arterial Blood Pressure and The Central Arterial Blood Pressure Using Invasive Method

Background and ObjectivesThe monitoring of the central blood pressure (CBP) is important in the treatment of the hypertensive patients. However, direct measuring of the CBP is not possible in outpatient management, instead, we usually cheek the brachial blood pressure (BBP). The aim of this study is to evaluate whether the BBP is accually reflects the CBP by performing invasive method. Subjects and Methods12 normotensive and 59 hypertensive patients who underwent coronary angiography revealed no significant stenosis were enrolled in this study. The invasively measured CBP, pulse pressure and pulse wave velosity (PWV) were obtained and the BBP was measured simultaneously. ResultsIn the normotensive patients, the mean of the systolic BBP was not statistically different from that of the systolic CBP (130.08±7.14 mmHg vs 125.42±15.97 mmHg, respectively, p=0.279). However, the mean of the diastolic BBP was significantly higher than diastolic CBP (78.08±9.59 mmHg vs 68.00±7.42 mmHg, respectively, p=0.001). In the hypertensive patients, the mean of the systolic BBP was not significantly different from that of the systolic CBP (149.00±15.54 mmHg vs 153.93±17.81 mmHg, respectively, p=0.096). The mean of the diastolic BBP was also significantly higher than the diastolic CBP (88.61±10.86 mmHg vs 78.12±8.26 mmHg, respectively, p<0.001). In the normotensive patients, there was positive correlation between the systolic BBP and the systolic CBP (r=0.515, p=0.007), the diastolic BBP and the diastolic CBP (r=0.556, p=0.003). Both the systolic BBP (r=0.470, p=0.015) and the systolic CBP (r=0.418, p=0.034) showed positive correlation with PWV. In the systolic hypertensive patients, there was also positive correlation between the systolic BBP and the systolic CBP (r=0.716, p<0.001), the diastolic BBP and the diastolic CBP (0.481, p=0.001). However, both the systolic BBP (r=-0.017, p=0.914) and the systolic CBP (r=0.112, p=0.466) did not show positive correlation with PWV. ConclusionThe systolic BBP and CBP, the diastolic BBP and CBP revealed excellent positive correlation, although the mean of the diastolic BBP was higher than that of the diastolic CBP.
