Background: Although ambulatory blood pressure(ABP) measurement is regarded as the gold standard of measurement, home blood pressure(HBP) has seen increasing use in clinical practice due to easiness. However, evidence of the value of HBP in the prediction of target organ damage is still limited. We sought to assess the value of home blood pressure(HBP) measurement compared with ABP and office blood pressure(OBP) in prediction of target organ damage in hypertension.
Methods: Fifty-six untreated patients with hypertension(men : 40 and female : 16 , mean age, 47.5±13.1years) underwent measurements of ABP, HBP and OBP. The ABP was recorded for 24hrs, HBP was measured for 1week, and OBP was measured at least in two visits. All BP measurements were taken using automatic BP measuring device. The parameters indicating target organ damage were the left-ventricular mass index(LVMI) by transthoracic echocardiography, urinary albumin excretion rate(AER), carotid-femoral pulse-wave velocity(PWV), and carotid intima-media thickness(IMT).
Results: The LVMI was significantly correlated with HBP, 24hrs ABP, awake ABP and asleep ABP, but was not correlated with OBP (Table 1). The AER and PWV were significantly correlated with HBP(AER, 0.42/0.11(SBP/DBP; p<0.01 for SBP) and PWV, 0.45/0.07(SBP/DBP; p<0.01 for SBP)). In a multivariate regression analysis, age and systolic HBP were significant predictors of AER and PWV.
Conclusions: Our data suggest that HBP measurements are independently related to hypertension-induced target organ damage as assessed by AER and PWV. Therefore, it will be expected that HBP adds information of the cardiovascular target organ damage in hypertension.
