The Prevalence of Extracardiac Findings Detected by Multidetector Computed Tomography Performed for the Evaluation of Ischemic Heart Disease

Background and Objectives : Recently, multidector Computed tomography (MDCT) has been commonly used for the evaluation of coronary artery disease (CAD). In addition to information about the coronary arteries, these examination includes portions of the lungs, upper abdomen. The study was designed to evaluate the prevalence of extracardiac findings discovered during MDCT examination for evaluation of ischemic heart disease. Materials and Methods : From March 2007 to March 2008, a total six hundred twenty patients were performed MDCT study for evaluation of chest pain or dyspnea and enrolled in this study. A cardiac and non-cardiac findings were comprehensively evaluated by radiologist. Results The patients consisted of 306 males (49.4%) with a mean age of 66.3±8.7 years. Significant coronary artery stenosis was found in 41.5% of the patients. A total of 149 extracardiac findings were observed in 109(17.6%) of the patients. Extracardiac findings were found more commonly in patients without coronary artery stenosis (19.8% vs 14.4%, p=0.049). Commonly involved extracardiac organs were lung (35.6%), hepatobiliary system (22.8%), thyroid (19.5%), kidney (12.1%), spine (6.7%) and breast (0.6%). Of those patients, 50(45.9%) patients were done further diagnostic investigations and most of them were benign pulmonary nodule. Malignant disease were detected in 3(0.4%) patients (lung cancer, pancreatic cancer, thyroid cancer) of all patients. Conclusions : When we do the MDCT examination for the evaluation of the chest pain syndrome, the presence of extracardiac findings is frequent and should be concerned.
