Background and Objectives : Since the bare metal stent era, vessel diameter and lesion length have been considered as strong predictors of restenosis and adverse cardiac events. However, the effectiveness of drug eluting stnet (DES) with large diameter is not known. This study was performed to evaluate and compare the clinical and angiographic outcomes of three 3.5 mm diameter DESs, Sirolimus-euting stent: SES, Pacritaxel-eluting stent: PES and Zotarolimus-eluting stent: ZES, in routine clinical practice. Materials and Methods : From May 2005 to July 2007, total 399 consecutive patients who have single coronary lesion and underwent 3.5mm diameter sized DES implantation were registered in this registry from 3 hospitals. Patients were divided into SES (n=272), PES (n=85) and ZES (n=42) groups and compared angiographic outcomes at 9 months and major adverse cardiac events (MACE) of cardiac death, myocardial infarction and target lesion revascularization at 1 year. Results : Baseline clinical and angiographic characteristics were similar. Stent thrombosis was occurred in one patients of SES (0.37% of SES patients, 0.25% of total patients). At 1 year follow up, MACE occurred in 0.7%, 0% and 4.8%, respectively (p=0.03). There were two episodes of TLR in ZES group (4.8%). Follow up angiography showed binary restenosis in 2 (11.8%) ZES, 0 PES and 2 (1.4%) SES patients. Quantitative coronary angiographic findings were as table. Conclusion : Coronary intervention using DES in large coronary vessels showed more favorable outcomes than reported trials in smaller vessels. A higher TVR was noted in the group treated with ZES than SES or PES.
