Background:As per previous studies diabetic patients with optimal blood sugar control after percutaneous coronary intervention (PCI) have similar revascularization rates as compared to non-diabetics.Pts on Insulin may be a long-standing, complicated subset compared with pts on oral-agent and it's unclear whether this clinical difference of DM may affect PCI outcomes. Method:319 diabetic pts with definite history of diabetes and underwent PCI were divided into insulin group (n=141 pts, 223 lesions) and non-insulin group (oral hypoglycemic agent, n=178 pts, 299 lesions).Angiographic and clinical outcomes at 6 months were evaluated. Result:Both groups had similar baseline characteristics except insulin group had higher levels of fasting blood sugar (205.2짹88.2 vs. 163.4짹64.3, p=0.001) and HbA1c (8.00짹1.5 vs. 7.6짹1.4, p=0.03). At 6 months, no difference in binary restenosis , late loss , Q wave MI and repeat PCI were seen but angiographic % restenosis , total death and MACE were higher in the insulin group. Conclusion: Despite the absence of significant difference in angiographic outcomes between the two groups, insulin group showed higher incidences of total death and MACE. Meticulous blood sugar control and special cardiovascular care will be essential and critical for the better outcomes of insulin-requiring DM pts.
