Efficacy of Statin Therapy in Acute Myocardial Infarction with very Low LDL-Cholesterol Level in an Asian Population |

Background:Current guidelines recommend that the goal of lipid-lowering therapy in patients (pts) with coronary artery disease is LDL-C level < 100mg/dl. Whether routine statin administration in the acute myocardial infarction (AMI) setting with low baseline LDL-C levels in a series of Asian population would be beneficial or not is unclear.
Methods:Data were analyzed from the Korea Acute Myocardial Infarction Registry (KAMIR) from 2005 to 2007.pts with the baseline LDL-C levels less than 70mg/dL were divided into 3 groups according to the different statin therapy strategies: statin therapy initiated before and continued after admission (Group 1); newly started statin therapy after admission (Group 2); no statin therapy (Group 3). Clinical outcomes in 8 months were compared.
Results: Compared with Group 3, Group 1 and 2 had lower incidences of mortality up to 8 months.Total major adverse cardiac events (MACE) were lower in the Group 1 and 2 at 1 month but were not maintained at 8 months.Multivariate logistic analysis showed statin administration didn't independently influence the clinical outcomes including cardiac death (P=0.559),total death (P=0.743) and total MACE (P=0.988) at 8 months.
Conclusions:Routine statin administration in Asian AMI patients with baseline LDL-C levels less than 70mg/dL might not get extra benefit from statin therapy.
