Background:. The clinical correlation between carotid IMT and the severity of coronary artery disease (CAD) is relatively poor. The aim of this study was to test the relation between coronary restenosis after DES implantation, carotid IMT and Coronary Calcium Score (CCS) Method: Among the total 3041 patients performed carotid ultrasound, 956 patients (male: female=583:373, mean age=61짹11) underwent follow-up coronary angiography after DES implantation were dived into two groups [Group 1(n=861, male=61%, mean age 60짹11) no ISR group, Group 2 (n=95, male=67.4%, mean age=65짹9) ISR group]. ISR was defined more than 50% reduction in luminal stent diameter.. Also, Intimal thickness defined as high echogenic thickening at intimal area. CCS on MDCT was also compared between two groups. Result: Comparing between two groups, 1. Right mean IMT and media thickness were larger in Group 1 (p=0.023, p=0.048). 2. There was no significant difference of right and left maximal IMT, left medial thickness (table). 3. CCS on ISR group were higher significantly (427짹86 vs 1599짹482, p<0.05). After adjustment with age, CCS was the strongest predictive of stent restenosis. (odd 1.3, p=0.027, 95% CI:1.02-1.89) Conclusion: CCS compared to carotid IMT is better predictive marker for stent restenosis with DES implantation. Noninvasive measurement of CCS and carotid IMT including media thickness might be useful for the evaluation of coronary restenosis
