Experience in a novel low profile stent graft for thoraco-abdominal aortic endovascular repair

Purposes: To evaluate the safety, efficacy of the low profile stent graft for endovascular aortic repair and the feasibility of preclose technique for percutaneous access closure. Methods: Consecutive data were collected from a single center on a series of 16 thoracic abdominal aortic disease patients. Eight patients had TAD. Four patients had TAA. Four patients had AAA. A stent graft is packed inside 15F to 18F low profile and flexible introducing system for percutaneous and small and tortuous ilio-femoral artery. The preclose technique, especially for low profile stent graft, involves two 6F perclose proglide devices deployed in the femoral artery prior to insertion of the large diameter introducer sheath or devices and the closure of the arteriotomy by tying down knots of the proglide following removal of the sheath. Results: All stent graft were deployed successfully. No patients developed stent-graft kinking, thrombosis or prosthesis migration during follow-up. Cumulative endoleaks occurred in three patients, early after index procedure. one patient required second intervention to treat Endoleak during follow-up. Femoral Arteries of 15 patients (94%) were closed successfully with the preclose technique. Conclusions: Results of low profile stent graft for the treatment of thoraco-abdominal aortic disease demonstrated to be safe and efficacious at follow-up. Percutaneous closure of femoral arteries after low profile stent graft introducing system using preclose technique was feasible for hemostasis.
