A 37-year-old male with unstable angina was admitted to our cardiovascular center. The patient had Takayasu���s arteritis and aortic valve replacement with a metallic valve due to severe aortic regurgitation 7 years previously. the coroanry angiography (CAG) showed a 95 percent discrete eccentric luminal narrowing at the ostium of the large left anterior descending artery (LAD), large left circumflex artery (LCX) and 99 percent discrete eccentric luminal narrowing at the ostium of the large right coronary artery (RCA)(Figure A, B). Both subclavian artery and right superficial femoral artery were completely occluded. The diameter of each coronary artery was greater than 5 mm. We delayed percutaneous coronary intervention (PCI) because the disease was in an active stage. The patient was treated with prednisolone 60 mg/day for 14 days. The ostium of the RCA was implanted with large 5.0 �� 12 mm Paclitaxel-eluting stent was implanted. Two large Paclitaxel-eluting stents were implanted from the distal left main coronary artery to the LAD and LCX by the kissing stent technique (LAD: 5.0 mm �� 24mm, LCX; 5.0 �� 16 mm) (Figure C, D). The follow up CAG did not show restenosis or recurrence of coronary artery disease. In this case, the PESs were effective for treatment of Takayasu���s arteritis involving the ostium of the LAD, LCX and RCA 6 months after treatment. Long-term clinical follow up is required and the clinical outcomes of more cases managed with drug eluting stents (DES) to establish the safety and efficacy of DESs for Takayasu���s arteritis.
