Aortic Stent Grafting for Ruptured Aortic Dissection and Aortic Arch Aneurysm

Aortic rupture has a high mortality rate and is a medical emergency. An aortic stent graft for an aortic rupture is currently thought to be an effective alternative treatment. We report two cases with an aortic stent graft used for a ruptured aorta. Case 1: A 72 year old male patient visited our cardiovascular center with hemoptysis. The chest computed tomography (CT) and aortography showed a 9 cm aortic arch aneurysm 0.5 cm distal to the left suclavian artery and a hemothorax in the left lung (Figure 1 A, B). The patient refused surgery. We performed a carotid to carotid bypass in advance and two pieces of aortic stent grafts (40 mm * 10 cm, 36 mm * 10 cm, S&G biotech, Korea) were placed across the left carotid artery and left subclavian artery. Atelectasis and hemothorax were present in the left lower lung (Figure 2 A, B). A therapeutic bronchoscopy was performed to remove blood clots from the left bronchus. The follow up CT showed the aortic stent grafts, no endoleak and thrombus in the aortic arch aneurysm. The patient was discharged from the hospital without complication (Figure 3A, 3B). Case 2: A 76 year old female patient presented to our cardiovascular center with hemoptysis. The chest PA showed a hemothorax of the left lung. The chest CT and aortography showed a retrograde aortic dissection with a dissection flap in the descending aorta (Figure 4A, 4B). We performed separated aortic stent grafts (36 mm * 10 cm, S&G biotech, Korea) in the descending aorta. There was no endoleak (Figure 5A, 5B).
