Drug-induced bradycardia: incidence and short-term prognosis

Background: Beta-blockers, non-dihydropyridine(DHP) calcium channel blockers, and digoxin are considered a cause of bradycardia for which pacemaker implantation is not indicated. However, little is known about the incidence and prognosis of patients with drug-induced bradycardia. Objectives: The aim of this study was to determine how often bradycardia is really caused by drugs. Methods: In a retrospective study we analyzed 57 patients admitted to emergency department between March 2005 and July 2007 with the diagnosis of bradycardia not related to acute myocardial infarction. The cause and effect relation between bradycardia and drugs was defined according to the response to drug discontinuation. Mean follow-up duration was 7.0±6.6 months. Results: Of 57 patients (26 men and 31 women; mean age= 68.2±13.9 year-old) with bradycardia, 37 (64.9%) were receiving beta-blokcers and/or non-DHP calcium channel blockers or digoxin. The initial electrocardiogram showed sinus bradycardia (heart rate <40, n=10), sinus bradycardia with junctional escape beat (n=27), second degree atrioventricular (AV) block (n=1), third degree AV block (n=19). Patients with drug-related bradycardia more commonly had hypertension and diabetes. Serum levels of K, BUN, Creatinine, and body mass index were higher in patients with drug-related bradycardia. Sick sinus syndrome was more common in patients with drug-related bradycardia (78.4%), whereas AV block was more common in patients with bradycardia in the absence of drugs (60.0%). Drug discontinuation was followed by resolution of bradycardia in 48.6% of patients receiving medications, whereas spontaneous improvement of bradycardia occurred in 30.0% of patients with bradycardia in the absence of drugs. Bradycardia that was “truly caused by drugs” was found in only 31.6% of all cases. Beta-blokers were the most common culprit drug (83.3%). Conclusions: Bradycardia is commonly “related to drugs”. However, only one-third of bradycardia is “truly caused by drugs"
