Effect of biodegrable poly(l-lactide-co-ε-caprolactone) PLCL based Paclitaxel coated stent on ISR and late vessel complication in rabbit iliac artery model

Objective - In this study, Our purpose was to determine application of biodegradable poly(l-lactide-co-ε -caprolactone) PLCL for drug eluting stent (DES) and long-term effects of PLCL based paclitaxel loaded stent. Background - Approved DES products used widely in clinical area and coated by non-biodegradable polymer. But in long-term follow-up, remaining polymer induced late complication and in-stent restenosis(ISR). To solve this problem, biodegradable polymer coated stent was investigated and reported. Among biodegradable polymers, PLCL had a biodegradable and elastic property to apply for DES. We tested biodegrable poly(l-lactide-co-ε-caprolactone) PLCL based Paclitaxel coated stent in animal model. Methods - Stents were uncoated or coated with a PLCL alone or containing paclitaxel and coated with PEVA containing paclitaxel. Each stent was implanted in the iliac arteries of rabbits after balloon injury. Result - The iliac arteries in all groups didn`t occur acute thrombosis. Tissue response were examined 28 and 90days after stenting. Neointimal area of PLCL based Paclitaxel coated stent was not lower than other groups at 28 and 90 days. All groups showed approximate quantity of injury and inflammatory score at 90 days. PLCL based Paclitaxel coated stent did not appear serious inflammation as compared with PEVA based paclitaxel loaded stent. Conclusion - Although PLCL based Paclitaxel coated stents had not good effect as well as approved DES, PLCL based Paclitaxel coated stents did not occur late complication that appeared in other biodegradable and non-degradable polymer.
