The position and configuration of the septum are primarily determined by using the transseptal pressure gradient. Changes in the position and geometry of the ventricular septum occur when there is an acute or chronic imbalance between left and right ventricular loading conditions. We will illustrate the pathophysiology, various cause, and the clinical significance of paradoxical sepatal motion revealed by cardiac MR Imaging.
