Impact of Final Stent Dimensions on Long-Term Stent Patency Following Zotarolimus-Eluting Stent versus Bare Metal Stent Implantation: A Serial Volumetric IVUS Study

Background:Stent underexpansion remains an important risk factor of restenosis following drug-eluting stent(DES)implantation.This study was to assess the predictive minimum IVUS threshold of post-procedure minimal stent area(MSA)for long-term stent patency following zotarolimus-eluting stents(ZES)compared to bare metal stents(BMS)implantation.Methods:382cases(ZES:210;BMS:172)with available volumetric serial(post-procedure and6-to8-month follow-up)IVUS examinations were analyzed.Percent neointimal volume was calculated as(neointimal volume/stent volume)*100at follow-up.According to previous physiological study,long-term adequate stent patency was defined as minimal lumen area≥3.0mm2at follow-up.Minimum MSA threshold was determined based upon slope analysis of the relationship between post-procedure final stent expansion and long-term adequate stent patency.Results:ZES had a lower neointimal volume index(volume/length)and%neointimal volume(1.26±0.87mm3/mmvs.2.39±1.28mm3/mm and17.3%vs.32.8%;all for P<0.0001,respectively).Follow-up minimal lumen area had a significant positive correlation with post-procedure MSA in both groups(ZES:r=0.76and BMS:r=0.47;all for p<0.0001).The minimum IVUS threshold of post-procedure MSA for long-term adequate stent patency was6.0mm2for ZES and6.5mm2for BMS.Conclusion:Post-procedure final stent expansion accounted for up to85%of ZES patency and67%of BMS patency.Stent underexpansion should be avoided since MSA<6.0mm2for ZES and<6.5mm2for BMS is associated with a steep decrease in follow-up stent patency
