Is There a Difference According to the Stent Types in Large Coronary Artery Intervention?

Background: Drug-eluting stent (DES) is generally known as smaller late loss than Bare metal stent (BMS). This might produce smaller incidence of adverse cardiac event rate after DES implantation. In terms of the bigger is the better of BMS, the clinical effect might be decreased in large vessel intervention. We would like to know whether there is any difference in adverse cardiac outcomes according to the types of stent in large coronary artery intervention. Method: Two hundred seventy two patients (BMS 104 /113 lesions, PES 41 /43, SES 127 /136) were consecutively enrolled since April 2003 to July 2005 in two interventional cardiology center, who had large reference vessel diameter between 3.5mm to 4.0mm. All the patients were divided into three groups: group 1 SES, group 2 PES, group 3 BMS. 12 months adverse cardiac outcomes and 6 to 9 months angiographic follow-up results were analyzed. Results: Major adverse cardiac events for 1 year were 3.0 % in group 1, 4.7 % in group 2, 12.4 % in group 3 (p=0.012). Other data are in the following table. 12months clinical follow-up was available in 99.3 %, angiographic follow up in 51.0 %. Conclusion: Drug-eluting stent showed better late outcomes than BMS in the large coronary artery intervention. However, there was no difference in the incidence of cardiac death. The type of DES was not affected in long-term clinical outcomes in large vessel intervention.
