Outcomes of Culprit Lesions with Plaque Rupture Following Bare Metal Stent Implantation in Patients with Angina Pectoris : A Serial Intravascular Ultrasound Study

Background:Coronary culprit lesions with plaque rupture have been treated with a variety of coronary interventions.However, it is unknown whether the presence of plaque rupture affects the restenotic process after coronary intervention.Methods:142 patients undergoing coronary BMS implantation were enrolled in retrospective analysis.Case selection was based on the availability of IVUS and QCA examinations at baseline(pre and post intervention) and at followup.Serial comparative analyses included qualitative and quantitative features of the culprit lesion as well as reference segments.Plaque rupture was defined as an intra-plaque cavity in communication with the lumen in the presence of a residual, disrupted cap.Patients were divided according to the presence/absence of plaque rupture.Results:Baseline patient demographics were similar between plaque rupture(+PR) and no plaque rupture(-PR) groups.Quantitative IVUS analysis showed higher rates of positive remodeling, and larger vessel and plaque areas in the +PR compared to -PR(P<0.001).At followup(7.2±2.6months),no statistically significant difference was observed between the 2 groups in either QCA or IVUS measurements.Conclusion:Culprit lesions with plaque rupture exhibited larger plaque mass and higher rates of positive remodeling at pre-interventional IVUS examination.When treated with BMS, however, the absence/presence of pre-intervention plaque rupture was not found to affect the rate or severity of instent restenosis in these culprit lesions.
