Background: Recent clinical trials recommend high dosage statin therapy in patients with acute coronary syndrome. There had been some concern that statin therapy might affect steroid hormone biosynthesis which is related with breast cancer. This randomized trial was performed to investigate whether intermediate dose statin therapy affects the steroid hormone level. Method: This study enrolled 30 men with aged 40-66 who had admitted with acute coronary syndrome and a serum total cholestol level of 196-308 mg/dL. The men were randomized into three groups; control(C), atorvastatin 10 mg (A10) and atorvastin 40 mg (A40). Their cholesterol and hormone levels were measured at baseline and after 6 weeks of treatment. Results; Atorvastatin effectively lowered plasma concentrations of total cholesterol (C, A10, A 40; 226±34 mg/dL, 151±35 mg/dL, 135±12 mg/dL, respectively, p<0.001) and LDL-cholesterol (C, A10, A 40; 153±27 mg/dL, 87±30 mg/dL and 75±12 mg/dL, respectively, P<0.001) after 6 weeks of treatment. Compare to the baseline, plasma cortisol level decreased in 83% and testosterone increased in 90% of patients after 6 weeks. But, there was no difference of cortisol (C, A10, A 40; 10.9±4.7 g/dl, 10.3±2.9 g/dl and 10.8±2.7 g/dl, respectively, P=0.920) and testosterone (C, A10, A 40; 4.2±1.8 g/ml , 3.9±0.7 g/ml, 4.2±1.2 g/ml, respectively, P=0.818) among three groups after treatment. Conclusions: We concluded that acute coronary syndrome transiently changes the steroid hormone levels and atorvastatin doses not effect on the steroid hormone.
