The Prevalence of Electrocardiographic Left Ventricular Hypertrophy and Relation to Other Cardiovascular Risk Factor in General Population : Based on the Large Prospective Cohort Study

Background and Objective: In general population, an electrocardiograhic left ventricular hypertrophy (ECG-LVH) is an important cardiovascular risk factor and the detection of ECG LVH is important to evaluate cardiac health status of general population. But there were few data for Korean general population, especially by prospective cohort study. The objective of this study was to examine ECG-LVH prevalence and relationship to other clinical profiles in general population based on the prospective cohort study. Method: We used a community-based prospective cohort of 1519 adult subjects from Korea Rural Genomic Cohort Study (KRGC study). Thorough biochemical and clinical data were recorded for all of the participants. Standard 12-lead electrocardiograms (ECGs) were recorded in all patients with a commercially available equipment. LVH was identified by the core laboratory using criteria based on the Cornell voltage×QRS duration product(CVD): (RaVL+SV3)×QRS duration > 2440 mm×ms in men and (RaVL+SV3+8 mm) ×QRS duration > 2440 mm×ms in women. An additional acceptance criterion was introduced based on the Sokolow-Lyon voltage(SLV) : (SV1+RV5 or V6) > 38 mm. Results: Cohort subjects are 627 males and mean age is 54.8±8.4 years. The distribution of current cardiovascular risk factors are cerebrovascular attack(2.3%), hypertension(25%), congestive heart failure(1.5%), hypercholesterolemia(4.5%), smoking history(32%) and DM(10.9 %), metabolic syndrome(36.9%), respectively. ECG-LVH by SLV was present in 8.4% and by CVD in 15.3%. In hypertensive patients, ECG LVH by SLV was present in 8.2% and by CVD 15%. By use of logistic analysis, presence of ECG LVH by SLV was predominantly associated with body mass index, hemoglobin, whereas presence of CVD was predominantly associated with higher hs-CRP. There were no significant differences of clinical profiles (age, BMI, proportion of other risk factors) between the patients with ECG-LVH and without ECG-LVH. Conclusions; The prevalence for LVH was not relatively low in general population based on our cohort study. Also, Korean general population who meet Cornell voltage-duration criteria and Sokolow-Lyon voltage criteria are associated with other clinical status.
