How could we detect restenosis in the drug-eluting stent era : Role of Myocardial SPECT

Restenosis is one of the most important factors to worsen patients’ prognosis after PCI even it is asymptomatic. Currently, the diagnosis of restenosis largely depends on the symptom or non-invasive stress test such as myocardial SPECT. In the DES era of low restenosis, the predictive values of these parameters are supposed to be much lower than BMS era, but, it is not well known. In this study, we tried to determine diagnostic values and indications of myocardial SPECT to detect restenosis. Method and Result: We prospectively enrolled 526 consecutive patients who underwent PCI with BMS or DES. All of them recommended undergoing myocardial SPECT and follow-up coronary angiography (FUCAG) 6M after PCI. The patients who didn’t take FUCAG or who had history of CABG were excluded. Finally, 425 patients were entered into analysis. 30% and 19% of them had history of diabetes and myocardial infarction. 67% of them treated with DES. Diagnosis of myocardial ischemia on SPECT depends on the expert's reading. Angiographically significant coronary stenosis was defined as diameter stenosis >= 70% in the main coronary branches or presence of coronary stenosis causing PCI. The diagnostic values of angina symptom and myocardial SPECT to detect restenosis are presented in the table 1. In multivariate analysis, use of DES(p<0.01), absence of diabetes(p<0.01) and asymptomatic state(p<0.01) were significantly associated with low positive predictive value and prior MI(p<0.001) was significantly associated with low specificity. Conclusion: The diagnostic values of myocardial SPECT were affected significantly by various clinical situations. Clinicians should decide whether to use myocardial SPECT to detect restenosis in the context of clinical setting.
