Clinical and angiographic consequences after overlapping drug eluting stent implantation.

Background: The spectacular results after drug eluting stent(DES) implantation prompted treatment of diffuse long stenosis with multiple overlapping stents implantation. However, the clinical and angiographic consequences of overlapping DES implantation is not clearly defined. Method: We enrolled 103 lesions in 98 patients(mean age:61±9, M: F=62:36) underwent overlapping DES implantation intervention in Saint Mary’s hospital from Jan. 2004 to Jan. 2006. Clinical follow up was done via telephone interview, and follow up angiogram was performed after 6 to 9 months after DES implantation. Results: 1. All patients had clinical follow-up and average period was 15.2±4.6months. 2. 70 patients (68.0%) had an angiographic follow up and average period was 7.2±3.8months. 3. 78 lesions (75.7%) were treated with sirolimus eluting stent(SES)s, 21 lesions (20.4%) with palcitaxel eluting stents and 4 lesions (3.9%) with one SES overlapped one PES. 4. 72 lesions (69.9%) were left anterior descending artery, 6 lesions (5.8%) left circumflex artery and 28 lesions (27.2%) right coronary artery. 5. The average stent length was 55.82±8.96mm and average stent diameter was 3.064±0.24 mm 6. In stent restenosis was observed in 16 lesions (21.3%) and ISR type according to the Mehran’s classification was IA in 2 lesios, IB in 4 lesions, IC in 4 lesions, ID in 3 lesions, II in 1 lesion and III in 1 lesion. 7. The lesions with ISR had significantly smaller average stent diameter than those without ISR(3.085±0.23 vs 2.919±0.27mm, p<0.05). 6. Major adverse cardiac event was observed in 18 patients (18.3%), including no death, no myocardial infarction and 18 (18.3%) target vessel revascularization. Conclusion: Overlapping DES implantation is feasible in patients with diffuse coronary artery disease.
