More prominent magnetic dispersion in repetitive ventricular ectopy patients with cardiomyopathy

Background: Recently, repetitive ventricular ectopy are reported to be related with cardiomyopathy. Magnetocardiography (MCG) is useful to interpret abnormal ventricular depolarization and repolarization. This study was aimed to evaluate the abnormal ventricular repolarization pattern in repetitive ventricular ectopy patients with cardiomyopathy using the MCG.
Methods: The MCG findings of 42 repetitive ventricular ectopy patients (Male 13, mean age 47.2 ± 13.8 years) were evaluated. All patients did not have coronary artery disease, valvular heart disease, and hypercholesterolemia. We compared MCG findings between patients with (n=32) and without (n=6) preserved left ventricular (LV) function. Magnetic field (MF) and conduction density vector (CDV) map were evaluated from 40 ms before and after the peak of T-wave (Tp). MF map was classified into dipole and non-dipole, In CDV map, the number of vortexes was calculated.
Results: In MF map, non-dipole pattern was more common in patients without than with preserved LV function at 40 ms before (Tp-40) and after Tp (Tp+40). In CVD map, number of vortex was also higher in patients without than with preserved LV function at Tp-40 and Tp+40.
Conclusion: Repetitive ventricular ectopy patients without preserved left ventricular function showed more magnetic dispersion pattern of repolarization than patients with preserved LV function. The MCG could be the new noninvasive tool to evaluate abnormal ventricular repolarization pattern in repetitive ventricular ectopy patients with cardiomyopathy.
