Preliminary Report for the Sirolimus-Eluting Stent and Paclitaxel-Eluting Stent in the Korea Acute Myocardial Infarction Registry (KAMIR)

Background Percutaneous coronary intervention (PCI) by Drug-Eluting stent (DES) is useful in acute myocardial infarction. The aim of this preliminary report was to clarify the use pattern of DES in Korea Acute Myocardial Infarction Registry (KAMIR). Methods On-line registry of acute myocardial infarction at the web site of www.kamir.or.kr has been performed in 42 primary percutaneous coronary intervention (PCI) centers in Korea. Between Nov 2005 and June 2006, 2,641 patients who were implanted Cypher® or Taxus® was enrolled in KAMIR. The study group were divided into 2 groups: group I [1594 patients, 1169 male, 62.1±13.1 years: Sirolimus-Eluting Stent (Cypher®)], group II [1047 patients, 747 male, 63.3±13.3 years: Paclitaxel-Eluting Stent (Taxus®)]. Baseline clinical characteristics and mortality rate during 1month were compared between the 2 groups. Results Baseline clinical characteristics, risk factor of coronary artery disease and proportion of STEMI were not significantly different between the 2 groups. The laboratory finding and left ventricular ejection fraction were not significantly different between the 2 groups. The target coronary artery was different between the 2 groups [group I – left main (LM):left anterior descending (LAD):left circumflex (LCX):Right coronary artery (RCA)=26(1.6%):884(55.7%):214(13.5%):463(29.2%), group II – LM:LAD:LCX:RCA=10(1.0%):472(45.1%):180(17.3%):384(36.7%), p<0.001] but. lesion type according to ACC/AHA classification were not significantly different in both groups. Diameter and length of stents were not different but, number of implanted stents was different (group I: group II=1.5±0.8:1.7±1.0, p<0.001). The TIMI flow grade (TFG) and mortality rate were not different between the 2 groups. Conclusions Even though mortality was not different, Cypher® stent was more commonly used in LAD and the number of implanted stents was lower in Korea Acute Myocardial Infarction Registry.
