The Fate of overlapped zone according to kind of drug eluting stents in multiple stenting

Background: Drug-eluting stents(DES) can treat the diffuse long coronary artery lesions more effectively than bare metal stents to some degree. For lesions extending beyond the capacity of a single stent, multiple overlapping stents are deployed. However, DES raise considerable concerns over local toxicity given that drug and/or polymer concentrations are likely to be significantly higher at sites of stent overlap. So we investigated whether there is difference for the changes of overlapped zone of multiple stents according to the kind of DES. Subjects and Methods: We included 52 patients who have been treated with 2 overlapped DES and have received the follow-up coronary angiography at 6 month later. We took the quantitative coronary angiography at the time of both coronary intervention and follow-up coronary angiography. We divided the subjects into 3 groups(group A, B, C). Group A(N=17) was treated with 2 Cypher stents, Group B (N=20) with 2 Taxus stents, and Group C(N=15) with Cypher and Taxus stents. Results: Baseline demographic factors(age, sex, BMI, left ventricular ejection fraction, and coronary risk factors) were similar in the 3 groups. Reference vessel diameters of 3 groups were 3.05±0.65, 2.99±0.35, 3.13±0.31 mm, respectively, p=ns). The lesions of all subjects were B2/C in modified ACC/AHA lesion class. There was no significant difference for vessel location of lesions among 3 groups. Total length of overlapped stents were 51.2±10.1, 54.7±8.5, 53.6±7.3 mm, respectively, p=ns). There was no in-stent restenosis in all subjects. As we compared late loss of overlapped zone of 2 stents among 3 groups, we found no significant difference according to the kind of DES (0.23±0.16, 0.28±0.17, and 0.27±0.15 mm, respectively, p=ns) Conclusion: With limitation of absence of intravascular ultrasound data, there was no significant difference for late loss of overlapped zone of multiple stents according to the kind of DES.
