Clinical outcomes of Drug-Eluting Stent Implantation in Left Main Coronary Artery

Background: The safety and efficacy of percutaneous coronary intervention in unprotected left main coronary artery (LMCA) are still a matter of debate. Methods: We retrospectively analized 6 months angiographic restenosis and 8 months clinical outcomes of DES implantation for LMCA stenosis in 98 patients (133 stents) who had sirolimus-eluting stent (SES) (87 patients, 121 stents) and paclitaxel-eluting stent (PES) (9 patients, 10 stents) between July 2003 and May 2006. Results: LMCA stenosis was classified to ostial (29 patients, 29%), body (18 patients, 19%) and Bifurcation lesion (51 patients, 52%). Bifurcation techniques for distal LMCA included crossover stenting in 23(45%), T stenting in 3(6%), kissing stenting in 20(39%) and stent crushing in 5(10%). Two hospital death (2%) developed without any other major adverse cardiac events (MACE). During follow up period, 2 cardiac deaths (2%, myocardiac infarction) developed and 8 patients (8%) received TLR (2 patients in CABG, 6 patients in repeated PCI). The total MACE was 12.2%. Follow up coronary angiography was performed in 65 patients(66%) and angiographic restenosis rate was 0%, 9% and 20% in ostium, body and bifurcation lesion, respectively. Conclusions: Drug-Eluting Stent Implantation for LMCA stenosis appears relatively safe with regard to complication and is effective in preventing restenosis.
